Provider I.D. Code

I.D. codes are individual to each provider in the directory. One I.D. code is allowed for each provider at the time of initial registration. The I.D. code is composed of a combination of numbers and letters. The I.D. code is required for a directory participant (provider) to enter or change certain types of information on his or her page in the directory. The provider must enter the series of numbers and letters exactly as entered during the initial registration for access to be successful. Disclaimer

I Lost My I.D. Code
Each provider is responsible for keeping your individual I.D. Code safe.
In case of lost or forgotten I.D. code, there are two ways to retrieve your I.D. Code:

  • First, Please try the ID retrieval function.
  • If you are not able to retrieve your I.D. Code using the above method, please contact Mental Health Referral at the following address. We will send your I.D. code to your office address listed in the directory by U.S. mail. We will not be able to give out your I.D. code by phone or e-mail.
